AKC number is necessary. Pay with cash or check (w/ driver’s license) at
time of appointment.
NOTE: All dogs will have their eyes dilated; dilation
lasts a few hours. It takes 15 minutes for dilation drops to be effective.
A CERF form must be completed which can be done while dog is dilating. Info
needed on the form: a)Date of birth, b) registered name, c) registration number,
3) owners name and address, 4) tattoo or microchip number if applicable.-
(Scanner to check for microchip numbers if needed is available.) A copy of
the CERF form will be provided to owner; owner must submit CERF form, with
the appropriate fee, to CERF, address on the back of the form

Orthopedic Foundation for Animals cardiac certification forms can
be downloaded from their website at www.offa.org under
the heading “OFA Applications” and subheading “Congenital Cardiac”. Please download
and complete the OFA application form as well as the AzVS
information form and bring with you to the clinic. The
completed OFA form will be provided to owner; who must submit OFA form, with
the appropriate fee, to OFA, address on the form.
